^^^ We booked it all separately. I bought the flights back in November (?) from Frontier- they fly direct from Denver to San Jose. Then I did some research on TripAdvisor and various websites on where to go within Costa Rica and where to stay... we ended up staying at the Korrigan Lodge, a small B&B in Punta Uva which is near Puerto Viejo. The only activity we "pre-booked" was white water rafting on our last full day; everything else was booked a day or two before through the bed and breakfast owners.
I'm not sure exactly how much everything cost- probably around $1200 for the flights and B&B. Then I figure another grand or so for meals, activities, etc while we were down there. Meals are pretty cheap- most dinners were around $40 (including a couple of drinks), and I think we spent $80 once. I'd guess we spent somewhere around $2300-2500 total.
Oh, and $10 for the stuffed animal I bought for my niece.

Have you driven around the metro lately? (there's no laughing smiley here?)

When I was in Honduras the roads have no guard rails on the "high" passes, nor are there lines in the road in most places. I think anyone can drive too. That was... interesting.
